The Ministry of Education has provided direction to school boards to have policies and procedures in place to support children and students with certain medical conditions in schools. The policy, known as PPM 161 – Supporting Children and Students with Prevalent Medical Conditions (Anaphylaxis, Asthma, Diabetes, and/or Epilepsy) in Schools.
Ensuring the health and safety of children and students with medical conditions is essential to promoting well-being, and requires a partnership among families, members of the school community and community partners, including health care professionals.
Parents are requested to complete the fillable one page HWCDSB Student Medical Action Plan for Transportation as per PPM 161 for students with medical conditions including asthma, diabetes, epilepsy and allergy/anaphylaxis. This is in compliance with the Accessibility for Ontarians with Disability Act.
To facilitate compliance to PPM161, your child will receive a blank form at school, as well a copy of the form is available on the school web site.
Please return the completed form to the main office of the school by Wednesday December 19, 2018.
